Assist Life Stage, Transition

Life is full of changes. Some are exciting, some are challenging – and many are overwhelming until one receives the proper help. We are a registered NDIS provider; thus, we are offering Assist Life Stage and Transition supports so that the participants themselves can move forward in making essential changes in life with confidence, clarity, and practical suggestions.

Our purpose is to assist you in processes of change – to offer support, structure, and the necessary level of aid, to help you make informed choices, develop independence, and move at your own speed.

What is Assist Life Stage, Transition?

Assist Life Stage, Transition is an NDIS capacity building support which enables the participants to overcome significant life transitions and focus on the future. This is not a short-term aid but rather an aid that seeks to build skills, confidence, and resilience to help in the long-term autonomy.

Such supports may be helpful during transitional times, such as a change of living plan, academic, occupation, family, or support. The idea is to get you more adjusted to change, more  organized, and get you to feel like you have more control over your life.

How We Support You

Our Assist Life Stage, Transition is goal-oriented and person-centered, which forms our basis. We use the time to understand what is important to you and assist you depending on your needs, preferences, and NDIS goals. Support may include:

  • Guidance through major life transitions, such as the move to a new home, starting or changing education, employment, or community roles
  • Help to develop self-confidence and independence through building day-to-day life skills
  • Help with decision-making, planning, and problem-solving
  • Assistance to understand how to manage and cope with daily routines and activities.
  • Support during changes in family, living, or care arrangements
  • Building confidence to engage with community services, supports, and activities
  • Mentor and coaching to pursue individual, social, or practical goals

Through all the support is given in a respectful, practical, and participant-led way- empowering you to be the leader of your own way.

A Supportive, Person-Centered Approach

We understand that change is a potentially frightening prospect, especially in the face of numerous events experienced at the same time. Our approach is calm, flexible, and encouraging. We focus on:

  • Listening to your needs and your priorities
  • Breaking significant changes into manageable steps
  • Aiding you to get on your own feet, and at the same time, be there when you require guidance
  • Working collaboratively with families, working with carers and other supports where necessary

Our mission is to make you feel strong and empowered, and not in a hurry or overwhelmed.

Who Is Assist Life Stage, Transition For?

This support may be right for you if you:

  • Are experiencing or preparing for any significant changes in life
  • Wish to build confidence, independence, and life skills
  • Need systematic support to work towards a personal, social, or practical goal
  • Have funding under Capacity Building – Life Stage, Transition to your NDIS plan

We support NDIA-managed, plan-managed, and self-managed participants.

Supporting Your Journey Forward

You do not have to go through change alone. You are either leaving a new stage of your life, adjusting to new circumstances, or planning your future. We will accompany you and will help you, hand-in-hand, to follow you, support your adjustment, and provide constant guidelines.
